\nResponsibilities:
\nThe Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:
\nCompletes the MDS, CAA’s and care plans within regulated time frames
\nAssesses resident through physical assessment, interview and chart review
\nDiscusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
\nReviews information from hospitals, consults, and outside agencies and uses such information in the completion of the assessment and care planning
\nCoordinates, identifies, and/or initiates significant change MDS
\nIs prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintain PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
\nRemains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements
\nQualifications:
\nRegistered Nurse, RN
\nAANC certification a plus. RAC-CT
\nKnowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred
\nExperience as an MDS Nurse
